Primary Things to Bring On the Day of LEPT
1. Notice Of Admission (NOA)
If nawala ang NOA, pwede ka mag email sa PRC office kung saan ka nag file ng application for exam. Mag request ka ng bagong NOA. Or magtabong ka kung ano gagawin pag nawala ang NOA.
2. Official Receipt
Kung nawala ang OR mo, ayos lang kasi nasa NOA mo naman ang OR number mo.
3. Completely filled out Annex A
4. Completely filled out Annex B
5. RT-PCR Result OR Certificate of Quarantine
Again, as of March 28, 2021, either/or po sya. Alin man sa dalawa pwede mo dalahin basta hindi ka kabilang dun sa mga group of individuals na listed sa Memorandum Order number 68 like healthcare workers, may direct contact o exposure sa mga covid-19 cases, etc.
6. Two or more Mongol No. 2 pencils
7. Pen/s with BLACK ink only (for the information sheet)
8. A meter-stamped window envelope
9. A long brown envelope
10. A long transparent plastic envelope (non-colored)
